All About Me form help!

My All about me form was downloaded off the Nottingham county childminding website and has a page with 7 hexagonals on it for the areas of learning. Do I look at development matters and write in anything the child can do already from that or do I just look at what the child can do and write it in my own words and cross reference from development matters to see which area it covers. If that makes sense. (its for my new mindee's starting points, he's 12mths old. Im going round and round in circles reading everything on how to do obs and planning and still cant get my head round anything. Im more of a learner by looking at examples how things are done. I feel so frustrated because I just want to start it and present it nicely as well. If anyone has anything I could look at I would be very grateful, I don't want to copy just get some ideas.Thanks

Right, my description of an All About Me Form is one that you give to the parents before the child starts which tells the childminder about their eating, sleeping, moving/handling, toilet stage etc. That's your starting point from the parents.

I have attached my summary sheets from my local authority. One is 0-3 years, which focuses on the 3 prime areas of learning, another is 3-4 years which brings in the 4 specific areas of learning and the other is a 2 year old summary assessment. You will see they are all similar and you don't have to write a massive amount. I do my own starting points after 5 weeks of the mindee being with me.

I agree that the All About Me Form should be completed by/with the parents. Therefore, I think that you should keep the jargon to a minimum and put things in plain English.

Thank you for your input, I have filled in a individual needs of the child form which has the majority of the things on it that are on the all about me form, but as I said there is also a form which has on it the areas of learning. Do I not need to starting points for all the areas of learning? Thanks

Before you start planning for your child's next steps you need to observe 'what he can do already'...some call it 'starting points' others 'all about me'...check this on the Development Matters guidance and then plan your next steps with the parents, observe the child by taking into account they develop differently and are unique in their learning.
