How to word a permission to leave mindee with an emergency person.

Hi there, just after a spot of advice. I am going to ask parents to sign a permission form incase an emergency crops up. Are there any words etc I should/need to include on it?

Rather than asking permission it is better to get parents to sign to say they are aware of the requirement that you have to follow.

Trouble is if parents said no, they weren't happy to give permission then you would be stuffed.

Better inform rather give them the option as it is statutory.

I think I understand that Sarah, I unfortuneately either have to leave them for half an hour with an emergency person, or not have them, trying to help the parents out by inconvieniencing them the least. x x
