I used to use NCMA ones but now for everything - existing marks/injuries, injuries or bumps in my care and meds I use a duplicate or triplicate book.
I simply put the date, the childs name the details of what I have done, seen, given and sign it and leave space for parent to sign. I have a copy for parent and then a copy for the childs folder. There is a useful index at the front of a duplicate or triplicate book so you can write the childs name and nature of the problem on the index.
Works for me and saves me storing lots of different forms - PLus a duplicate book is small enough to take out and about with me so I can write it up whilst I am out and about.
triangle sandwiches are better than square ones...
