    Default Accident or Emergency

    Good Morning everyone

    Does anyone know where I can get an "in case of accident or emergency" card from.

    I am looking for a card that a parent carries with them in case they are in an accident which lets people know that their child may be with a childminder and the card has the childminder details.

    I have seen one issued by the NCMA but cannot find one on their website.

    I made mine and laminated, one credit card size for the wallet and a smaller one for key ring

    I use the back of my business cards, hole punched and threaded onto a key ring.

    They are easy to replace if things change and I shred them if a child leaves.

    I carry them with me whenever I am out... along with emergency paperwork etc

    I do them myself and laminate.

    They have my contact details and the contact details of my 3 emergency contacts. It is credit card sizes and fits in the parents wallet.

    these are mine. first names only and month and year as birth dates. i put my own meds and conditions on mine.

    i laminate them and they are on a key ring in my changing/everything else bag


    photo of each child on the back....babies sometimes look the same!!!


    ofsted loved them esp the ones for mums and dads to keep in purse or wallet

    Probably didn't explain very well.

    We have the ones that WE carry giving ours and the childrens detail should we be in an accident, it is a card for Parents to carry so that should they be involved in an accident OUR details are on the card so that WE can be contacted, one of our parents carries one of these cards from the NCMA but I am unable to find them

    Hope this makes better sense.

    The ncma pack (link above) contains one card for childminder and 4 for parents - there is another pack here http://www.shop.*************/four-ext...ds-1145-0.html with extra parent ones

    I sometimes find with these things, that in the time I've spent trawling the internet to find something, I could have created it myself!
