HELP PLEASE have I got everything I need?

I'm meeting with a parent in a couple of hours to fill in all the paperwork. It's for 2 brothers and I use MM contracts so I only need one contract, I have a child info form for each child, a welcome pack for Mum to take away, an 'all about me' form for each child, and a permissions form for each child.
I'm a bit worried i've missed something from the permissions form though. I have permission for photographs (and the various places photographs may be used eg on the newsletter), taking child in the car, seeking medical assistance, treatment or advice, first aid, sun cream, garden equipment eg swing and slide, sharing info with other settings as they aslo go to nursery.....anything else I need???

sounds good to me - I don't use MM contracts at the moment so don't know about them and I do tell parents that there might be more permission forms to add over time! just to cover myself
