Yes you do need it as it is a requirement to gather that information. There is one in the free downloads which I made last year. If you go to the top of the forum, between the little faces there is a link "Free Resources" click that, then from the 'file' choose the downloads and it is on the right hand side, somewhere near the bottom, there is also some information done by Sarah to download too which might help you.
