Please has any one an All About Me form I can pinch.

I'm just wondering how different it is to the Initial Child Assessment form I have that someone has given me which has bits under the headings Unique child, positivie relationships etc. It has a column for observations, which I've started filling in with date and comment.

Do these initial assessments go into the living journal folders or are they kept seperate.

I think I've been messed around so much by different parent's that I've forgotten what I'm supposed to be doing

There's an early years passport in the free downloads section which Angel shared that is one way of recording info about a child - don't know if that will help you.

Any information or assessments about a child I keep in their learning journals.

You can look at mine if you like and maybe adapt it if you wish. I get the parents to fill it in then I put it in the child's LJ and use it as a starting point.
