    Default questionaires!

    hello!

i know this is asked time and time again! but, i want to change my parents questionaire, as the one i use is very 'in depth' and one parent who continues to write me essays each time ! i was thinking that a simplified questionaire would be good, but i can't think what to ask!

there was a super really short questionaire posted a while back and i thought i had saved it, but i can't find it

    can anyone help?

    thanks

    xxx
