Fire drill help needed before I pull all my hair out!!

I made a lovely log book to keep track of all my fire drills and I cant find it so I am trying to do another one, but my minds gone blank.. On my form so far I have dd/mm/yy, name of children present, exit taken and thats it, just cant think of what else I should have on there but I know there should be more, can anyone give me a clue....

Mine also has-

*Which room was everyone in. Ie- I could be in kitchen mindees in dining room
*How long it took us to do the drill
*a space for me to me to explain how we carried out the drill ie any minded carried etc
*any improvments

Thanks for that, I am struggling to fit it all in to a grid, would anyone have a template to hand, I am so useless at designing grids to fit everything on Many thanks xxxxx

I just write it in a diary and say what happened how we discussed it, I then date and sign it.
