consent form for non prescribed medicine.

Can anyone help me find a download for a consent form for non prescribed drugs, I have got everything else but that one. I have just read another link saying about calpol so I might not need it unless it is used for other things like cough medicine???
Any help appreciated as new childminder with head full of alsorts!!!

I was going to say Bromley as well Cherry!

There is a good one there... and since EYFS doesn't start until September and we are all waiting with baited breath for some common sense amendments, the Bromley form is great for adapting and using for now
