    Default Child Questionnaire

    Does anyone give their older mindees a questionnaire to fill in about yourselves?? How happy they are when they come to you, what they would like? etc etc

    I have looked everywhere but can't find one.

    I was thinking school age children and for younger ones they could draw a picture of their time with you.
