    I finally got my health declaration form back from doctors and its only cost me £16, I was really scared reading all the high prices on here, so one last thing to worry about ...yay

    Grrrr 70quid mine cost!
    Glad you got it cheaper though, it's a ridiculous amount of money for a signature and a sentence scrawled on a bit of paper declaring you fit to work x

    i just spent 30 quid for a letter that was 2 lines long after an enquiry on my health declaratiaon form for a job glad yours was a bit cheaper honey xx

    OFSTED recently asked me to do a health declaration. Don't know why as been minding 13yr ...it cost me ...NOTHING ... I couldn't believe it

    Mine was £15 and took a month! No writing just a signature and stamp took a month !!!!!

    Mine was £12 and took three days! I recently had to have a private medical and that was well over £100 and he doctor had the cheek to make me wait 20 mins after my appointment time! That annoyed me!

    Mine was free two (last Sept) I was expecting a huge fee so I was really surprised when there was no charge! And pleased

    I'm in Wales as first the GPS receptionist told me it was a £50 charge to be paid on collection then when I collected I was told there was no fee which was fab.
