    Default Accident forms

When an accident form has been filled in I have that I sign and date it and so does the parent/carer. Do you have to give them a copy? I dont have a photocopier or scanner though would have to write the form out twice and have us sign both lots, would that be ok?

I use the ncma ones and they are self carbonated so yes parent does have a copy.

My printer photocopies - I always give them a copy for their records.

You could buy carbon paper from a stationery shop

Ok thanks, I will get some carbon paper for now. I intend on buying a printer with a photocopier but I need to earn some money first. ha ha
