    Default variation documentation ...how do you put yours on paper

    im about to ask DH to design me a simple format to record any variations that i may need ,needed one today due to parent failing to confirm change of days so i filled the space with an adhoc ,parent text late last night to say is it still ok for H to come !!! i had 4 under 5 but decided as it was for continuity that it was fine ,problem is that i was unprepared, does anyone have a template or document that i could look at /adopt or any ideas of what i should include in one,
    im planning on putting name and ages of children, date of variation (or period of variation required) ,time variation required from /to,reason for variation ,anything i may need to do differenly on the day .
    have i missed anything obvious..had a very long day
