Post HELP - Risk Assessment Form

I have spoken to Ofsted about my Mother in Law staying with us for 3 days and they told me that she wouldnt be CRB'd as she is not permanently living with us. As adviced on here they said I need to do a Risk Assessment and recommend I send a letter to the parents, which I intend to do.

I am wanting to put the following:-

    Date:
    Visitors Name:
    Length of Stay
    Reason for Stay
    Letter to Parent
    Not left unattended with Mindees

I am struggling to come up with a layout for the form and wondered if anyone could help me?

    Thanks
    Ann :

Yes mostly it's about not leaving visitors unattended with children but you might also find it useful to go through your emergency procedures with your visitor and talk to them about where to put hot drinks / bags that might contain tablets etc.

Hope you have a lovely few days
