Parent Questionnaire/survey

Evening all,

Does anyone have a parent questionnaire or survey (whatever you want to call it) that you wouldnt mind sharing with myself?

I really want to get one sent out to all my parents asap but I am absolutely crap at designing these types of things and each time I try, I panic that the questions I'm putting on it are either stupid, too basic/simple or over the top etc

Glad you did - I have redone mine to ask the parents three things they like best about the setting and then three things they would like to see introduced/could improve the setting, then any other comments, it's short and sweet and I'm hoping it will get some nice responses!
