    Default Permission forms school age children

    I was wondering what permissions may be needed for school aged children?
    I have made a set of forms but realise they are very much under-5's orientated.
    I don't want to overload parents with paperwork which may not apply to them.

    Hi. To be honest I have never thought about using a different one for after school mindees, I use my normal contracts (morton michel) for all of my mindees regardless of their age can't miss anything out then!

    I have some shorter 'all about me' forms for my schoolies - taking out the nappies stuff unless the child is disabled. I very much tailor them for the family.

    I also have a behaviour agreement which I work on with them in the first week or so to make sure they are clear about my expectations and sanctions etc.
