parent questionaire - Scotland

hi there am new to the forum i have been a registered childminder since nov 07 ,the care commission are looking for me to do a questionaire for the parents to fill out but i don't know where to start,has anyone done one that could give me guidance thanks

If i had read this earlier i could of sent you a copy of what i ask,but just finished packing for my move to scotland general questions about how parents feel their child is doing,any ideas they have that may help you with their child,their opinion on themes,someone else will come along with more information am sure

I ask questions in line with our self assessment so that I have good parent participation when filling in self assessment, another way to do it is to base it on the Care Standards as that is what we should be working towards. I also include more general questions like how happy to talk to me about concerns, how happy are you with meal options, is your child happy with activities.
