Risk Assessment - Identified a Risk

This is a bit of a stupid question - but it is stll early on a Sunday morning so might just get away with it.

I have all risk assessments in place, ie for outings (walking/car/bus etc)

Whilst walking to school the other day there were lots of road works down one particlar stretch. They had made a pedestrian walkway but they didn't make it wide enough for my double pushchair and we had to walk on the road anyway. The next day I decided to walk a different route to school to avoid this again. But where do I record this???

Certainly not a stupid question.
Maybe you could record this in your Accident/Incident Book, I know it is not an accident but it is and incident that may affect the well being of the mindees.

I would record somewhere on the risk assesment - if there is no space availabe on front of sheet then I would record on reverse

my RAs are on A4 sheets. I would write it up and staple it to the sheet as a review

Date and note on your regular RA
