Default Adding to RAs, additional risks

I have RAs in place for all parts of my setting and outings. I wondered what you do when you see an additional risk. For example in the garden, I've realised there's an extra risk. Should I add this to the RA and then re-date it so its renewal date changes. Or do I make a note seperately and the date for renewal stays the same?
Wondered what other people do with this.
    Thanks

All I do is add to/amend the original document - and date the new additions.

It shows its a working document with thought gone into it

my RA's are not going to win any beauty contests lol

I just add any new additional risks to the risk assessment and sign and date it in pen, no need in wasting ink and paper printing out another one.

I have a daily risk assessment so if i see a risk in the garden i note it down, date it and say what I am going to do to alleviate the risk.

If the risk is always going to be there I just add it and date when add it. If only temp - ie fence blown down I just make a note in my diary state what I did about it, and put in diary when fixed.
