    Smile risk assessment template

    I am currently trying to do some risk assessments and keep looking at different layouts which seems to be confusing me. Is this the correct sort of lay out?

    - date of RA
    - the hazards
    - who could this cause harm to and how
    - risk level
    - what is in place
    - any further action needed
    - review date

    can anyone please help?
    also hazards like washing up liquid in kitchen cupboard, can you say that what is in place, is to keep children out of the kitchen unless supervised by adult? Is this enough?

    Looks good to me but have a look through similar questions ask previously, I think where RA is concerned no-one likes to reply.
    I have asked friends (cms) for a copy of theres to Redraft for myself, Its such a mindfield makes my head hurt

    you have the correct format, it is easier though to do a room by room one, then the garden etc and any other risks you can think of.
    Outings exactly the same, if you got to a place on a regular basis put down all the risks etc then just date underneath - no further risks or list any hazards found

    I have shared my RA's in the past - I use a different system to some.

    Not everyone likes it but I know others use a similar system.

    My Mrs O (Oct 10) loved my RA's (not saying all will - as all inspectors have different opinions )

    mine is very basic -what, who at risk, risks & action so i think ill use the gloucester one as looks quite indepth. ofsted would have freaked at mine probably!

    Hi just wanted to know if I was dong these right?

    I have this so far and needed to update my risks to look a bit better!

    Date written and then to be reviewed annually or when I notice anything different!

    Do we need to put an actual date in the review bit

    Mine say

    What the risk assessment is for

    Completed on (date) by (full name)

    Column headings:

    Hazards
    Who might be harmed
    What is already in place
    Any further action needed
    Frequency of checks

    Date of next review
    Date review completed

    Yes you need to put an actual date in the review column (RA's should be reviewed at least once a year)

    Having just completed my risk assesment course this is the template we were all given, mine are very similiar so not changing them!

    I am finding these very helpful thanks for the link. Just one question; where it says at the top of the form 'putting the assessment into action' and underneath it has columns with 'by whom', 'by when' and 'date completed', do these columns need filling in at the initial risk assessment or do we fill these columns in if any further action is needed or on review?

    this form is more for nursery managers or people in charge of large operations.

    The manager does the RA then asks other members of staff to put things into action.

    You wouldn't necessarily do that as it's just you... you would just fix things.

    However you might be planning to fix things in the future such as upgrading something or buying something ... then you could use that section and you would be the person 'by whom' actioning the comment.

    The RA template from the south glouc website is almost identical to the one that the NCMA recommend we use - well in Bucks anyway!!

    I purchased sarahs book - very informative ......... but i still struggle with risk assessments - I now i shouldn't but i find them pointless at times.
