Personnal self evaluation

Has anyone got a example of how to set a simple one out?

I want to do something so i can show how i change my setting but just cant find a way to record it, i know i can update my sef online but i want my own simple one :)

For something simple, I would use the 5 Every Child Matters outcomes...

How I meet them... how I intend meet them better... :D

Make a note in your diary.

I have a notebook I jot down things like that in - might just be scribbling to anyone else :laughing:
