Please can anyone advise on what paperwork they have a parent complete for holiday club. I will be having 2 children as a one off for feb half term and not sure what paperwork i should have mum complete!

We need to collect the information stated in the Eyfs - that's normally on a contract and child information form which includes allergy / food requirements :D
