Slightly confused about Health Declaration Form

I hope someone would be kind enough to clarify when/where I send off the Health Declaration Form. I'm just completing the online application at the moment

I've had it back from my GP and unsure whether I need to send it back to Ofsted immediately or whether they need it at some later stage.

You send it to Ofsted - the Picadilly Gate, Manchester address that is on the form :D
