Im going to call my buisiness Catherynne's Childminding and have got that header on the top of all my policies but do i need to put my full name at the bottom like the risk assessments ???

Well done on getting organised! I have a header at the top so it's clear I've not copied them, then I sign and date at the bottom with a review date yes.

Yes - sign and date and review date. Although header makes it clear to most - some inspectors do like it to say written by because you may have asked someone else (family member prehaps) to do it.

I've got a box at the bottom of mine for 'signed' and 'dated'

I also got carried a bit away when making them, and put a header and a watermark in.
