Parental Responsibility and Legal Contact

I think this is the best idea.

I haven't asked my parents to say what they don't have... just what they do have. Where have tick boxes come from? To be honest I'm not sure they are necessary...

If someone has legal contact they will know about it because they will have to have been to court to get it. If they don't have it and haven't been to court I would say that they should just be able to leave it blank like you've put here Pauline.

The tick boxes were on a form that my local NCMA workers had drawn up and asked the questions something like 'do you have parental responsibility' and 'legal contact'. I am now going to draw up my own forms using the wording Pauline suggested.
