quality of care reports

CAN ANYONE POINT ME IN THE RIGHT DIRECTION PLEASE?
I'M LOOKING FOR A TEMPLATE OR A QUALITY OF CARE REPORT TO READ SO I KNOW WHERE,WHAT AND HOW TO PRODUCE MY OWN REPORT.
I AM IN PANIC MODE SO REALLY NEED HELP!!!!!!

Who has asked you to make this report?
Is it a local authority thing?
Never heard of one...
If it is the LA asking then I'd ask them for a template or some support. I am not sure if they should even be asking for extra things like that... why?

I think this is a Welsh thing? Am I right? If so, you might have more luck posting your question in the CSSIW part of the forum.
