Parental Permission Form Question

I have a parental permission form which bullet points a list of things. I have now included in my policies and procedures about mobile phones, etc. Should I have a bullet point about this in my permission form too?

I don't ask permission from parents to use my phone - I tell them that I might use it to take photos when we are on outings if I have forgotten my camera.
