    Default child record forms

    Do I have to use NCMA forms or can I make my own child record forms???

    I've got a mum coming to fill in paperwork for afterschool care and need more record forms and there is no way they would arrive before they come so would my insurance be void or anything like that if I make my own ones?

    I use my own too, they're more like an information sheet than a binding contract so can't see it would be a problem.
