    Default Address to send Health Declaration

    Finally my GP has completed my health declaration so I am now in a position to send it to Ofsted.

    I know they don't like things sent by post so does anyone know the email address to send it to?

    Or if I've got it wrong, please let me know the correct postal address.

    Any help is much appreciated.
