Default Assessment - anyone have a form for this pretty please?

Hi ya I had one of those RAG assessments and the only thing that I don't seem to do is do a summarise assessment each term pulling altogether my observations etc ?? I just wondered if anyone has a form or a way that they lay it out to help me get a grip of what I'm meant to be doing ?? I would be so grateful!!

Mine is just an A4 sheet with a box for each area of learning, one for next steps and one for parents comments - quite easy to make on the computer.

I do an I depth summative written report a couple of times each year but also use much shorter more user friendly ones inbetween they have areas of learning with clip art relevant to each one and then I write a few sentences about children's current development and next steps
