If you pm me your email - I'll send you mine - I use them when I do termly reviews with parents...it's just a v positive way of showing how a child is progressing - you don't have to use them - how you present your paperwork is up to you! HTH

The above is how I use mine - I find them very easy to fill out using previous obs & looking back through daily diaries
