all about me sheet

Has anybody got a good all about me sheet that goes in the childs learning journal please. Ime looking for one that tells you what the child can do when they first start in your setting. Ive seen one in the free downloads but its not really what ime after.

Have you tried a Google search? You might be able to use bits from different ones tailored to your needs

Thank you - l like what you've done with the form - specific questions etc.
I find I ask the parents these things but forget when the time comes (have 12 on my books now so its memory)
May well use some of these ideas.
