learning journey template?

I was thinking of one where you state the child's age, photo and what they like doing etc. Thank you

There might be a page or two you like from this link, as well. I use somehting very similar to the All About Me for the front page of my lo's observation ring binder. Haven't needed to yet but was thinking I'd do a new one every so often, to really show progression.
