Ido a summary assessment. I right a paragraph on each area of learning, state which age bracket they fall into and an area for improvement.
At the end I write a general paragraph about the child and how I wish them well.
Very much like the two year old report we are going to need to do from September. Think there is a link in the free down loads or try the early years website for examples.
Like fifi says it's a summary of what the child can do and any areas they are currently working towards.
It's just like the annual or biannual summary reports you are probably writing anyway.
I add a few photos to liven mine up... it's normally given to the parents to pass on to the reception teacher unless you already have a relationship with the school.
Try and get parents to contribute and the child can say something about how they are looking forward to starting school and anything they are a bit nervous about.

Our school has their own document that they usually give to parents but could be filed in by cm too so might be worth asking. Otherwise just a summary like Sarah says.

I was given a transition report form by my DO - it is printed by the County Council for EY providers. It may be worth asking your DO if they may have one?
Hi, its jas here, writing transition report for the reception,
you can write about the child what he/she Has achieved at your setting. Also child can do work independently in what areas and basic overall report or call it assessment about the Child that you might have done in your setting I.e. observation and evaluation and next step.
Less likely the school will accept as they will hav to assess themself with the school's curriculum as they will have to put the child in the group level according to their abilities. It's quiet challenging for the childminders where we stand. Good luck I hope this is useful to you.
