    Default child record forms from MM

    hi ive just registered and am ordering my paperwork from MM tomorrow but had a query re the child record forms and what information is on them?

    is it the name, address, gp, emergency details etc?

    as im just compiling an all about me form and wasnt sure if i needed to add this to that or if it was part of the child record form from MM

    Name child is known by....
    Address......
    Tel....
    Name of persons that collect the child....
    Password...
    Name of persons with legal contact...
    Special diet requirements/restrictions...
    Other relevant info...

    Carer 1 info.... Carer 2 info.....

    Emergency contacts....

    Emergency procedures..... (in the event of an emergency who do I contact first)
    Plus permission to seek emergency advice/treatment...

    Medical Records....

    Parents sign and date.
