    Default 2 year check

    What are the different ways we can "present" this information to parents and to others. Previously ive always done these in Tapestry but i cant this time and looking for another document or way to write it. Any help appreciated :-))

    I tend to do mine as a narrative document.

    I start with background info - child's name, DoB, when they started with me and which days/times they attend. Also anything else relevant, eg, language spoken at home, in receipt of 2 year old funding.

    Then under each heading (Communication & language, physical development etc) I write a bit about what the child is doing, what we're working on and what parents can do to help at home.

    At the end I say whether or not there are any areas for concern.

    Parents really like a written report and I've had good feedback from any health visitors who have actually taken the time to read it!

    Thanks so much Mouse, how do you share it with parents? Do you upload it anywhere or "just" email them a copy/print out for them?

    We have integrated reviews here, so we send ours to the health visitor team.
    I email it to parents first so they can add any comments, then forward it to the HVs. I'm not convinced they ever read them, but the parents like them!

    We have 'shared reviews' here, our LA has a form that we can use or you can use your own. I use the LA one as it's simple and easy to use which the parents read and sign then we upload it on to our LA portal.
