    Default Existing injury forms

    I'm sure it has been asked before, but can we make our own existing injury forms?

    I don't really like the ones that my insurance company provides.

    Yes, there is no requirement to use any particular form that I know of.

    I've heard of an insurance company saying you had to use their contracts (although I don't know if that still applies), but I've not heard of any saying you have to use their existing injuries form. I suppose you could always double check with them

    I use my own. It's made up of bits and pieces I've liked from other forms.
