    Default How to record...

    Existing injuries...

    I have a new little one who has an existing injuries - nothing suspicious, just the typical minor bump for an energetic toddler. I always give parents a blank existing injuries form when they start to us if and when they need it. This mum hasn't used it, but has informed me in writing in the child's diary as well as mentioning it to me at drop off. Is this enough or would you also get her to complete a form? (I'm sending more blanks home with her today in case she lost the one I gave her at the start).

    I have taken a photo of the diary entries and could print that out, but I'm thinking I should also fill out my regular form for my folder so that they are all in one place.

    I ask them to write it on a page at back of diary, and I then put a copy in medical file, and also a note in attendance register!

    Only if they've signed it in back of book. Otherwise I just date it and sign it myself.

    I have a home-made form. Parents tell me verbally, or note in diary, and I then fill in the form. Parent signs on collection. I file the form.
