    Default 2 year progress check

I have seen lots of examples for recording the 2 year progress but where is the best place to get the blank templates for us to fill in please? Thanks

My LA do a form that we can choose to use or not that has tick boxes at the bottom saying - filled in as part of the settling process, as ongoing review, as a 2 year check or as a transition report. So we fill out the same form and just tick to say why it was completed. I think it's great as it all covers the same thing anyway and then the 2 year check doesn't seem like such a big deal. If you use termly reviews (or however long) could you use it to do your 2 year check too?
