mine is hand written but goes a bit like this:
PSE - self awareness and self confidence: J comes in very happily each morning, separating from her parents with ease. She happily selects activities and welcomes praise for her achievements

Feelings and behaviour: J expresses her feelings well and is accepting of comfort and distraction. She is beginning to know when she has crossed a boundary and shows this through her behaviour. J is able to respond to no thank you.

I basically lifted statements from her age range on the profile!

A Unique child Early years Foundation Stage
    Learning and Development Summary
    Name ************ Date 13.09.2012 Age 30m

    A child learning
    Playing and exploring
Finding out and exploring; Playing with what they know; Being willing to ‘have a go’
    Active learning
Being involved and concentrating; Enjoying achieving what they set out to do; Keeping on trying
    Creating and thinking critically
Having their own ideas; Making links; Choosing ways to do things

J is interested in small world play; especially train sets and Duplo toys. He can name several Thomas the tank engine trains. He makes tower constructions and also wheeled constructions which he joins together – he sometimes combines the two and makes tall and long constructions. J enjoys being outside and likes climbing at the park as well as the slide and other equipment. J points out shapes he sees when we’re out and about ‘triangle tree’ and ‘circle window.’ Personal, Social and Emotional Development
Self-confidence and self-awareness; Making relationships; Managing feelings and behaviour
J is independent and likes to do things for himself, although he readily asks for help when he needs to. He is initially shy when meeting adults and children for the first time but soon becomes confident with people he sees regularly. J plays with and alongside other children and manages to resolve disputes quite well. He co-operates well within boundaries and routines mostly, sometimes needing encouragement at tidy up time.

    Communication and Language
    Listening and attention; Understanding; Speaking
J communicates confidently; he has a wide vocabulary and narrates while playing. He describes what he can see and tells me ‘I go in the garden, see combine harvester’ when playing pretending games. J responds to instructions and understands ‘we’ll finish lunch first, and then go in the garden’. J understands that objects are different sizes and uses small, big and bigger when describing things. Physical Development
Moving and handling; Health and self-care
J is doing very well with toilet/potty training and is mostly dry during the day. He can use the toilet and potty but is still learning to poo in the potty/toilet. Joseph manages to dress/undress for the toilet and asks for help when required. J runs confidently using his whole foot and jumps with 2 feet. He climbs stairs using alternate feet and comes down using 2 feet per step. J mostly uses his right hand when drawing circle and line shapes, using a tripod grip. He enjoys making shapes with his body – stars and triangles.

Next steps to support learning and development:

    • Continue to support J’s interest in small world activities by supplying a wide range of small world play – trains and tracks, vehicles and garage, farm and animals, rockets and astronauts, dinosaurs
    • Link J’s interest in finding shapes in the environment to activities – making pictures using shapes. Offer a variety of puzzles
    • Continue to offer a range of physical opportunities – park, soft play, garden
