thanks for reading: I'm applying for a variation and have downloaded the free form but for some reason the 'grid' hasn't loaded properly. I didn't realise until I'd written loads and would rather not have to start all over again :(

So... Does anyone know what the headings are in the 3rd 4th and 5th columns of the grids at the end of the form please?

3rd= How long have you cared for this child(months)
4th=Full time education*? Y/N . * says at bottom of page four or five year olds attending maintained nursery classes etc( let me know if you need this bit too)
5th=Own Child? Y/N
