I am about to hand in both parts 1 and 2 of the CSSIW application form, part 2 giving evidence of paperwork for every one of the 21 Standards. Do I need to include any parent permission slips with this, or just policies stating that I will get parents permission for such things, ie emergency treatment, etc. I have done so much paperwork already that I dont really want to do things which arent necessary, and that can be done at a later date.

It is a little while since I completed mine but I think I just put that I would use NCMAs permission forms. I didn't actually include them with the app
