There is a new fact sheet from Ofsted (Feb 2011) which clarifies the type of accidents and incidents that must be reported to Ofsted.
It also notes what needs reporting to the local child protection agencies. Note they use this wording through the document rather than 'Local Safeguarding Children board'.

Thank you for that Sarah, its saved in my files and just printed a copy for my ChildProtection folder.
Safeguarding as a term was returned to Child Protection again almost as soon as the present Government came to power.

My LSCB is still called that but in some areas the name has been changed.
The Govt website also talks about child protection more than safeguarding.
I am hedging my bets and still referring to both. :D

I would interpret the following as you would not have to
If you are on the Early Years Register you must tell us about a child accident,
injury or death on the premises while the child is in your care.4 It does not require
you to tell us if any other person has had such an incident, or if the incident happens
off the premises, or if it happens to a child not in your care, for example with their
parent after the end of any care period on their way out of the premises.
