Hi my fees are written up as "Contracts & Fees" I have written a section about how I meet with parents to disguss details & answer any queries thay may have.....blah blah blah
Which contracts i use (NCMA), what the contracts cover, how often they are reviewed, how long settling in contract is & how much notice is required on either side.
What my fees include & what parents are asked to provide.
What my usual working hours are & days that i do not work (eg xmas, new years etc)
Then i list my fees
Hourly, full day, late collection & how frequent its charged, sickness child & me, occasional days off child & me, annual holidays how many weeks & fees for child & me, school holidays & bank holidays
hope this helps :)

This is a really important one... you need to state how and when you want to be paid, what you will do if you are not paid on time, mention bouncing cheques and your expectations for a happy working relationship with parents. Then list your fees...
complaints how do i word this
I use the complaints stuff from Ofsted -
http://www.ofsted.gov.uk/assets/Internet_Content/Shared_Content/Files/compchild_revfinal.pdf
Tbh, you could do your own, but if a complaint is made against you, you are that scared, you just want it to be right... so you use their things anyway!
