hi, i just made my own on headded A4 and printed x2 copies something like this;-

child's name....
fees are now due
Date eg July 2010

arrears/outstanding N/A or eg(£30)

amount for month July............£400

total....................................£430

Break down

wk 5th................................£100
wk 12th..............................£100
wk 19th..............................£100
wk 26th..............................£100
extra day xxxxxxx £30
total..................................£430

you could also add any futher information such as bank holidays or say when payments need to be paid by.. and any "Late payment fees etc
