To register my husband do I fill in form EY 2 then if he gets accepted do I then fill in form to ask for number increase.

When I applied for an assistant I did the request for more mindees at the same time.

Did you use the EY2 form and put the request on the extra piece of paper included for further information? Not sure where to put the request:panic:

i did both at the same time and when the came out to see me the said tat was ok, i can have 2 more under 5 with my partner with me, hope this helps
