Does anyone have an all about me booklet that they use for older mindees and would be willing to share?

I have one for my younger mindees for the parents to complete but would like to do one for older mindees to complete themselves. I have an 11yr old due to start in a few weeks and thought it would be a good way of finding out what she likes etc.
