Do you all use NCMA/MM accident forms or do any of you use your own??? I seem to always be ordering something from NCMA and was wondering whether to start using my own ones. I would get parents to sign the form on collection of child and keep it in my accident folder.....would this be ok???

i think youd need to check with ncma insurance ( if you use them). i know you need ncma contracts , not sure about accident forms.

The only thing I don't use from NCMA is the child record form - however, I have used their form to type my own, word for word. But, like you, I was forever ordering them, and there are only 6 in the book. I just photocopy it and give the copy to the parents. I still use the NCMA contracts, attendance book (but this might change soon) and the accident, medication etc forms.
