I filled in the accident book at nursery today for the 3 yr old mindie i look after, he had fallen back on his chair and scratched his arm on the bin had 2 scratch marks like he had been scratched by a cat. I signed the form and was given no copy. Is there anything i should do to document this for my own records?
Do i fill in an existing injury form and who would sign it?

I would fill in exsisting injury form and put what happened at nursery and get parents to sign.

I always get the nursery to photocopy it,so that you can give the copy to the parents.
